Address formatting

How would I do this: Take inividual text fields like ''Firstname''?''Lastname'' ''Address1'' Address2'' ''City''?''State'' ''Zip'' and combine them into one field like this (assuming user leaves out ''Address 2'' and I don't want a black line): Firstname Lastname Address1 City, State Zip
